Detailed Guidelines for Completing Form No. 03 – Application for Issuance or Extension of Work Permit

Form No. 03 is the official template issued by the authorities and is used for both the initial issuance and extension of a work permit for foreign workers in Vietnam. All information must be accurate and consistent with the approval letter for foreign labor demand, employment contract, supporting documents on qualifications, and consular-legalized foreign documents

Form No. 03 is the official template issued by the authorities and is used for both the initial issuance and extension of a work permit for foreign workers in Vietnam. All information must be accurate and consistent with the approval letter for foreign labor demand, employment contract, supporting documents on qualifications, and consular-legalized foreign documents. Any inconsistency may result in delays or refusal. The following explains each item as required:

(1) Name of the competent authority:


Enter the exact name of the authority responsible for processing the dossier, either the Ministry of Home Affairs or the provincial Department of Home Affairs, depending on the employer’s organizational type and location. Incorrect designation of the authority will result in non-acceptance of the application.

(2), (10) Type of employing organization:


Specify the legal form of the employer, such as: State sector; enterprises (state-owned, non-state, foreign-invested); non-public service units; foreign organizations; contractors; or other agencies/organizations. Correct identification ensures compliance with regulatory classification.

(3), (11), (12) Address of the employer and working location:


Provide the full administrative address in the correct order: house number, street; hamlet/village; commune/ward; district; province/city. The address must match the business registration certificate and the labor contract.

(4) Economic sector code (Level 1):


Enter the relevant code under Decision No. 27/2018/QD-TTg. This code must reflect the employer’s principal business sector and correspond with the worker’s job position.

(5) Conditional business lines (if applicable):


If the enterprise operates in a regulated conditional business sector (e.g., education, healthcare, cybersecurity, logistics), specify this clearly according to current laws.

(6) Category of foreign worker:


Specify whether the worker is a Manager; Executive Director; Expert; or Technical Worker. This classification must be supported by the worker’s qualifications and experience records.

(7) Job title and occupational code:


Complete according to Level-3 occupational codes under Decision No. 34/2020/QD-TTg. The job title must match the labor contract and the previously issued work permit (for extensions).

(8) Form of employment:


Enter the appropriate category under Clause 1, Article 2 of Decree 219/2025/ND-CP, such as: working under a labor contract; intra-corporate transferee; contract-based service provider; professional expert; technical worker… Incorrect classification is a common reason for rejection.

(9) Educational qualification:


Specify the highest obtained qualification: Doctorate, Master’s Degree, Bachelor’s Degree, or others. Documents issued abroad must undergo consular legalization and notarized translation into Vietnamese.
